360 Degree Feedback Introductory Offer

We offer an opportunity to get feedback on your performance by participating in our 360 degree feedback process. The steps are:

• You select a group of people to provide you with feedback (your raters). Please make sure that your raters are willing and able to participate.
• You register your raters on the web following the steps below.
• We will send the survey to all your raters. We will also follow up with reminders to get the surveys are completed.
• We will produce your feedback report and send it to you with a Leaning Guide to assist you interpret the feedback.

The Introductory Offer uses a standard survey.

The Introductory Offer is open to one individual from an organisation that has not previously used Full Circle Feedback. The aim of the offer is to assist organisations evaluate our 360 degree feedback process.

There is no fee for participants in our Introductory Offer.

Rater selection is very important. Please choose people who:

• You would "value" receiving feedback from
• Are in a position to observe you on a regular basis
• Will provide candid, constructive feedback.

You will need the email addresses for your raters.

• Complete and send us this form with some simple details
